For Club Managers
Replace paper waivers. Publish a consent form once, parents sign it online for each child, and your club keeps a tamper-evident record with a downloadable certificate.
Any form a parent needs to agree to: liability waivers, medical and emergency consent, photo and media consent, code of conduct, travel permission. Every plan, including the free one, includes consent forms and e-signatures.
Open the Documents tab of your club dashboard and create a form:
Once created, the form is shown to the parents in your club, ready to sign.
The parent who added the child signs from their dashboard, once per child:
Both the parent and the club can download a signature certificate afterwards.
Each signature stores an exact copy of the form text the parent saw, a fingerprint (hash) of that text, the typed name, the time and the device details, sealed with a cryptographic signature. If anyone changed the stored record later, the seal would no longer match. Editing a form never alters what earlier parents signed.
A form that has signatures can't be deleted, so the club never loses its evidence. Archive it instead: it stops being offered to parents, and its signatures stay available. You can unarchive it at any time.
Are electronic signatures on consent forms valid?
In the US, the ESIGN Act and UETA generally give electronic signatures the same effect as handwritten ones when the signer agrees to sign electronically, which Squad asks for. Check with your league or insurer if they have specific requirements.
Can I use our existing waiver?
Yes. Attach it as a PDF or Word file, or paste its text.
Do both parents need to sign?
No. One signature per child is collected, from the parent who added the child to Squad.
